Instructions
Please, complete the required information below with the name of the person who is the main contact, this person is usually the council president or treasurer. Then upload the filled forms as required below. Valid formats are PDF, Word and Excel (for roster). Maximun size per file is 500Kb.

1. Application for Re-Charter/ Contact Information Form
2. Annual Council Re-Charter Agreement;
3. Tax Compliance Checklist
4. Contact Information Release Form
5. Worksheet and Council Membership Roster
NOTE: Once the form is completed and the council forms are uploaded you will be redirected to the payment page to pay the calculated dues. You can also pay sending a cashier check or money order payable to LULAC National for the total remittance due. National will not accept dues for district or state. Remember to always keep a copy of all paperwork and checks for your records.
